I hereby grant the production team the right, but not the obligation, to use my name, voice and/or likeness and/or any simulation thereof and/or any photographs taken of myself by the production team, individually or with others, in connection with the production, distribution, advertising and exploitation of, forever and throughout the world in any manner and by any means whatsoever, for this production.

The production team will not be responsible to my legal representative or me for any loss or damage to my personal clothing or effects.

All wardrobe, set dressing, props and other articles furnished by the production team for the purposes of the movie shall be and remain the property of the production team and shall be returned promptly in good condition by myself. I acknowledge that I will be held responsible for any loss or damage, except for fair wear and tear for such articles loaned to me.
